Dr Kumar's training as a Fellow of the Royal Australian College of Surgeons in the plastic and reconstructive surgical specialty gives him the anatomical grounding to work at multiple tissue planes in the face, from the superficial skin to the underlying SMAS (superficial musculoaponeurotic system) — the fibromuscular layer that connects facial skin to the deeper structures and whose laxity is the primary driver of lower facial ageing. The standard facelift technique involves an incision that follows the natural skin creases of the face — within or just above the temporal hairline, transitioning to the anterior surface of the ear in the helical crease, around the lobule, and into the postauricular hairline — providing concealed access to the facial planes while leaving scars within or adjacent to the hair. Through this access, the SMAS is elevated and repositioned with suspension sutures or excision, restoring the upward and lateral vector of tissue support that has descended over time. The overlying skin is then redraped conservatively and trimmed, with all tension on the closure distributed to the SMAS layer rather than the skin surface — the key to achieving a natural rather than pulled appearance. Neck contouring is routinely addressed simultaneously: submental liposuction removes excess fat from beneath the chin and jowl, while platysmaplasty — plication or corset suturing of the paired platysma muscles through a small submental incision — corrects the platysmal banding and blunted cervicofacial angle that contribute to an aged neck appearance.
