This repositions the descended midface fat pad to its youthful position above the zygomatic arch and corrects the jowl by addressing the underlying structural cause rather than redistributing surface skin. Because tension is placed on the deep tissue rather than the skin, the closure is virtually tension-free, resulting in fine scars, a natural appearance at rest, and longevity of correction quoted at 10–15 years. The traditional SMAS facelift is offered for patients with moderate lower face and neck ageing where full deep plane dissection is not required; SMAS elevation and suspension corrects jowl formation and early platysmal banding without the extended recovery of the deep plane variant. The mini facelift is a shorter-incision procedure confined to the periauricular area, providing effective improvement for patients with early lower face laxity who are not yet at the stage of requiring a full surgical facelift — typically used in the 40s and early 50s as a first intervention.
