The closed approach, with incisions entirely within the nostrils, is used for more limited modifications where full exposure is not necessary. Dorsal hump reduction involves controlled osteotomy and rasping of the nasal bones combined with trimming of the upper lateral cartilages; the subsequent open-roof deformity is closed with medial and lateral osteotomies. Nasal tip refinement uses suture techniques and selective cartilage modification to control tip projection, rotation, and definition without aggressive resection that can lead to long-term structural compromise. For patients with a deviated septum or inferior turbinate hypertrophy contributing to breathing difficulty, septoplasty and turbinate reduction can be performed concurrently, addressing both the cosmetic and functional components in a single operative session.
