A carefully measured ellipse of excess skin is excised; the underlying orbicularis muscle may be partially thinned if it contributes to lid heaviness; and the eyelid fat pads may be partially reduced or repositioned if their prolapse contributes to the hooded appearance. The incision is closed with fine sutures that are removed within one week, leaving a scar that settles into the eyelid crease and becomes effectively invisible with time. Lower blepharoplasty addresses the lower eyelid, where the primary concerns are fat pad herniation causing puffy or baggy under-eye appearance, and tear-trough hollowing (the sunken concavity at the junction of the lower eyelid and cheek that creates a shadow and tired appearance). The transconjunctival approach — through an incision inside the lower eyelid with no external skin cut — allows repositioning or conservative reduction of the herniated fat pads without any external scar; fat repositioning into the tear trough hollow is preferred over simple removal to avoid a skeletonised under-eye appearance. The external (subciliary) lower blepharoplasty approach adds a fine incision just below the lower lash line when skin excision is also required. Blepharoplasty is frequently performed concurrently with facelift, brow lift, or fat transfer to the mid-face for comprehensive periocular rejuvenation.
