Eyelid Lift (Blepharoplasty)

Upper and lower eyelid surgery (blepharoplasty) by Dr. Cat Huang-Begovic to address hooded upper lids, excess upper eyelid skin causing visual field obstruction, lower eyelid puffiness from fat herniation, and under-eye hollows. Upper blepharoplasty removes excess upper eyelid skin and, where indicated, a small strip of orbicularis muscle. Lower blepharoplasty uses a transconjunctival approach (no external scar) or external approach to reposition or remove lower lid fat pads. Often combined with facelift or brow lift.

Eyelid surgery (blepharoplasty) at Dr. Cat Plastic Surgery addresses the periocular changes of ageing — the area around the eyes is often the first facial region where patients notice and wish to correct signs of ageing, as the eyelid skin is among the thinnest on the body and loses elasticity relatively early. Upper blepharoplasty addresses hooded or heavy upper eyelids caused by excess skin accumulation (dermatochalasis) on the upper eyelid platform, which may cause the eyelid to overhang the eyelash line, create a tired or aged appearance, or in advanced cases obstruct the superior visual field. The upper blepharoplasty incision is placed precisely in the upper eyelid crease — a natural skin fold that conceals the scar very effectively after healing. A carefully measured ellipse of excess skin is excised; the underlying orbicularis muscle may be partially thinned if it contributes to lid heaviness; and the eyelid fat pads may be partially reduced or repositioned if their prolapse contributes to the hooded appearance. The incision is closed with fine sutures that are removed within one week, leaving a scar that settles into the eyelid crease and becomes effectively invisible with time. Lower blepharoplasty addresses the lower eyelid, where the primary concerns are fat pad herniation causing puffy or baggy under-eye appearance, and tear-trough hollowing (the sunken concavity at the junction of the lower eyelid and cheek that creates a shadow and tired appearance). The transconjunctival approach — through an incision inside the lower eyelid with no external skin cut — allows repositioning or conservative reduction of the herniated fat pads without any external scar; fat repositioning into the tear trough hollow is preferred over simple removal to avoid a skeletonised under-eye appearance. The external (subciliary) lower blepharoplasty approach adds a fine incision just below the lower lash line when skin excision is also required. Blepharoplasty is frequently performed concurrently with facelift, brow lift, or fat transfer to the mid-face for comprehensive periocular rejuvenation.
