Breast Augmentation

Dr. Salomon performs breast augmentation using saline and cohesive silicone gel implants at his South Miami practice. Implants may be placed via an inframammary incision (the preferred approach, eliminating sensation changes at the nipple), periareolar incision, or transaxillary approach. Dr. Salomon, board-certified by the American Board of Plastic Surgery since 1999, offers both saline and cohesive silicone gel implant options, and works with patients to determine implant volume, profile, and placement based on existing breast tissue, chest wall dimensions, and the patient's lifestyle. Saline implants consist of a silicone shell filled with sterile saline solution. They tend to have a firmer feel than silicone options and allow a smaller incision, since the shell is filled after placement. Silicone (cohesive gel) implants have a softer feel often described as more similar to natural breast tissue. Modern cohesive gel formulations maintain their shape even if the outer shell is compromised. Regarding incision options, the inframammary incision (in the fold beneath the breast) is the practice's preferred approach, as it avoids any contact with the nipple-areola complex and eliminates the theoretical risk of altered nipple sensation. Periareolar and transaxillary (armpit) approaches are also available; the transaxillary approach leaves no scar on the breast itself. Placement above the pectoral muscle (subglandular) typically means a shorter recovery; placement below the chest muscle (submuscular) is associated with lower rates of capsular contracture in some studies and allows better mammogram imaging. Surgery is performed under general anesthesia and takes approximately 90 minutes. Most patients are comfortable returning to desk work within a few days. Lifting the arms above shoulder height is restricted for roughly ten days, and strenuous physical activity should be avoided for six weeks. Implants are theoretically long-lasting, though replacement is recommended approximately every 16 years per manufacturer guidance.
