Upper eyelid blepharoplasty addresses excess dermatochalasis — the redundant skin that accumulates in the upper eyelid as the skin loses elasticity over time. In advanced cases, the excess skin may fold over the lash line and impinge on the superior visual field, creating a functional indication for surgery in addition to the cosmetic improvement. The procedure removes a precisely calculated ellipse of excess skin and, where present, a small volume of herniated orbital fat from the medial pocket. The incision is placed precisely within the natural upper eyelid crease, leaving a scar that is invisible when the eye is open and concealed within the crease when closed. Results produce a rejuvenated, more open eye appearance that looks rested rather than operated. Lower eyelid blepharoplasty targets the pseudoherniation of orbital fat — the under-eye bags caused by weakening of the orbital septum allowing fat to prolapse anteriorly — as well as fine periorbital wrinkles and skin laxity. Dr. Jejurikar approaches lower eyelid surgery via either a transcutaneous (external) incision placed just beneath the lash line or a transconjunctival approach through the inner surface of the lower lid, which leaves no external scar. In patients where loss of volume rather than excess fat is the primary concern, fat repositioning or grafting may be used to restore the natural tear-trough contour rather than simply removing tissue. Blepharoplasty is frequently combined with other facial procedures at this practice — most commonly brow lift, facelift, or neck lift — to address the periorbital region in the context of overall facial rejuvenation. Standalone eyelid surgery is also performed as a focused improvement for patients whose primary concern is limited to the eye area.
