Each of these micro-injuries triggers the skin's natural cascade of wound healing: the initial haemostatic and inflammatory phase recruits platelets and growth factors to the site; the proliferative phase activates local fibroblasts to synthesise new collagen (types I and III) and elastin; and the remodelling phase over the following weeks and months reorganises these new fibres into a denser, more structured extracellular matrix in the papillary and upper reticular dermis. This fibrogenic mechanism is the basis of microneedling's clinical efficacy across multiple indications. For atrophic acne scars — particularly rolling and boxcar subtypes — the needling-induced fibrous remodelling breaks up shallow fibrous bands tethering the scar base and replaces the disordered scar tissue with newly organised collagen, progressively raising the scar floor towards the surrounding skin level. For enlarged pores, remodelling of the perifollicular dermis tightens the visible funnel shape of the follicular opening. For fine lines and early rhytids, new collagen deposition thickens the dermis and reduces the appearance of surface creasing. The micro-channels also transiently elevate transcutaneous drug absorption by orders of magnitude compared to intact skin, enabling active serums applied during or immediately after needling to reach the mid-dermis where they are most effective. Cassy uses this window to infuse hydrating and repairing serums during each session. Needle depth is adjusted zone by zone — 0.5 mm for the delicate periorbital area and fine surface texture concerns, and 1.5–2.0 mm for the cheek and chin regions where scar remodelling requires deeper dermal penetration. Topical anaesthetic cream is applied and allowed to absorb for 30–45 minutes prior to the session, making the procedure well-tolerated. Mild redness and minor pinpoint bleeding normalise within 24–48 hours.
