In patients where dermatochalasis is severe enough to obstruct the superior visual field, upper blepharoplasty may be assessed as functional surgery under Australian Medicare criteria, requiring a visual field test demonstrating the impact on peripheral vision; where this threshold is met, a Medicare rebate (Item 45617 or similar) may apply to the functional component of the surgery. Lower blepharoplasty addresses under-eye bags — caused by herniation of orbital fat through the weakened orbital septum — using either a transconjunctival approach (incision inside the lower lid, no external scar) or a skin-muscle flap approach (external subciliary incision) to reposition or remove the herniated fat. Lower eyelid procedures may be combined with skin resurfacing to address fine lines and crepey skin in the periorbital area.
