The pre-operative consultation is a detailed process in which Dr. Patel assesses chest wall dimensions, the existing breast tissue volume and envelope quality, nipple-areola position, skin tone, and the patient's stated aesthetic goals to arrive at the optimal combination of implant type, size, profile, and placement. Silicone gel implants — available in standard, moderately cohesive, and highly cohesive (form-stable, colloquially 'gummy bear') variants — are the most commonly selected option for their natural feel and shape retention. Saline implants offer the advantage of a smaller incision and allow fine-tuning of fill volume at the time of surgery. Implant size is selected in cubic centimetres in relation to chest width and the desired projection outcome. Projection profiles range from low through moderate to high and extra-high, and the right profile for each patient is determined by chest width and the desired visual projection. Placement options include the dual-plane submuscular approach, where the upper pole of the implant sits under the pectoralis major muscle providing additional coverage and a more natural upper-pole slope, and the subglandular (above the muscle) approach, which is better suited to patients with an adequate natural tissue envelope and results in a shorter, less uncomfortable recovery. Incision options include the inframammary fold — the most common, most precise, and most easily revised if ever needed — the periareolar, and the transaxillary approach. Breast lift (mastopexy) and breast reduction procedures are also offered, with mastopexy frequently combined with augmentation in patients who have experienced volume loss alongside ptosis. Recovery involves wearing a supportive surgical bra for several weeks, avoiding heavy lifting for four to six weeks, and gradual implant softening over three to six months.
