Botulinum toxin works by blocking acetylcholine release at the neuromuscular junction, temporarily preventing the small facial muscles that create expression lines from contracting fully. The result is a softening of dynamic wrinkles — lines that appear with facial movement — while at-rest skin texture and static wrinkles are minimally affected. The consultation that precedes the injection session is as important as the procedure itself. The physician observes your facial movement to map which muscles are driving which lines, assesses the depth of the lines at rest versus during expression, and plans the injection map and dosing accordingly. The goal is not immobility but calibrated softening: enough relaxation to smooth the expression lines without obliterating natural movement or producing an unnatural flat appearance. Individual anatomy varies considerably, so a standard millilitre or standard unit count applied the same way to everyone is an oversimplification the physician avoids.
