Laser skin resurfacing uses light energy to reshape the skin surface and stimulate new collagen and elastin formation in the dermis. At Klinika preventivní dermatologie the decision between ablative and non-ablative approaches — and between full-beam and fractional delivery — is made by the physician based on the patient's skin type, the concern being treated, the acceptable downtime, and the depth of change needed. Fractional laser delivery — the approach used here — divides the laser beam into a pattern of thousands of microscopic columns, each of which creates a controlled micro-wound in the skin while the surrounding tissue between the columns remains intact. This 'fractional' approach preserves islands of healthy, intact skin that seed the healing response, allowing faster recovery and lower risk of hyperpigmentation than traditional full-ablation resurfacing, while still triggering meaningful collagen remodelling. The depth of ablation within each column is adjustable and determines the result: shallower settings smooth fine surface lines, refine pores and even tone with minimal downtime; deeper settings address more established lines, acne scars and texture irregularities with a more significant healing phase of three to seven days. Before treatment the physician marks the areas to be treated and applies a topical anaesthetic cream (EMLA) for around 30–45 minutes, which minimises discomfort during the procedure. The laser handpiece is then passed methodically over the treatment area in overlapping passes; patients typically describe a brief heat sensation with each pulse. After treatment the skin is red and feels warm, similar to a sunburn; a cooling dressing or gel is applied in the clinic. Sun protection begins immediately on leaving and is maintained for four to six weeks post-procedure. For fine lines and mild textural improvement one or two sessions may suffice; for established acne scarring a course of three sessions is typically recommended, spaced six to eight weeks apart to allow the collagen remodelling to mature between visits. Full-face fractional resurfacing is from 4,500 Kč; targeted zone treatment is from 2,800 Kč per session.
