Non-surgical skin tightening addresses the root cause of sagging: reduced collagen and elastin density in the dermis, and in more significant laxity, reduced tension in the superficial musculoaponeurotic system (SMAS) beneath. Two complementary energy-based technologies are used at Klinika preventivní dermatologie, each reaching different depths and suited to different degrees of laxity. Radiofrequency (RF) tightening delivers electrical energy that heats the dermis evenly to a controlled therapeutic temperature of approximately 40–42°C. At this temperature collagen fibres contract immediately, providing an acute tightening effect, and fibroblasts are stimulated to produce new collagen over the following six to twelve weeks. RF energy does not penetrate below the dermis, making it well suited to surface and mid-dermal laxity — improving firmness and skin quality on the face, jaw line, neck and body — and its cumulative effect builds over a course of three sessions. The procedure is performed without anaesthetic and is described as comfortably warm, like a hot stone massage; there is no recovery period. High-intensity focused ultrasound (HIFU) converges ultrasound energy at precise focal depths — 1.5 mm, 3.0 mm and 4.5 mm — creating tiny thermal coagulation zones at each level. The 4.5 mm depth reaches the SMAS, the same tissue layer addressed by a surgical face lift but without any incision, triggering contraction and subsequent collagen remodelling that lifts and tightens more significantly than RF. HIFU is more comfortable with topical anaesthetic and produces a mild temporary soreness in the treated area, which resolves within a day. Results are gradual, peaking at two to three months as collagen matures, and often suffice for two to three years before a maintenance session is warranted. The physician selects the appropriate modality and number of sessions at a consultation that assesses the degree and location of laxity, skin quality and patient expectations. For mild laxity or as maintenance RF is preferred; for more pronounced sagging or jawline definition HIFU gives a more decisive result.
