Eyelid Surgery (Blepharoplasty)

Type

Eyelid Surgery

Dr. Rady Rahban performs upper and lower blepharoplasty (eyelid surgery) in Beverly Hills to address drooping upper eyelid skin that encroaches on the visual field, upper lid hooding, lower eyelid bags, and under-eye hollowing. Upper blepharoplasty removes excess eyelid skin and, where appropriate, a small strip of underlying orbicularis muscle and herniated orbital fat. Lower blepharoplasty addresses puffiness through a transconjunctival or external approach and may incorporate fat repositioning or fat transfer for volume correction.

Blepharoplasty (eyelid surgery) at Rady Rahban MD, Beverly Hills is one of the most commonly performed procedures at the practice, addressing the periorbital aging changes that have a disproportionately large impact on facial appearance: redundant upper eyelid skin that creates hooding, narrows the palpebral aperture, and can cause superior visual field obstruction; herniated orbital fat that produces lower eyelid puffiness and the classic under-eye bag appearance; and hollowing or tear trough deformity where fat has involuted, creating a shadowed, fatigued look. Upper blepharoplasty at the Beverly Hills practice involves a carefully designed elliptical excision of redundant skin and orbicularis muscle along the upper lid crease — the incision is placed precisely within the supratarsal fold so that it is invisible when the eye is open. Where herniated medial or central orbital fat contributes to upper lid fullness, a small fat excision or repositioning is performed simultaneously. For patients with significant lateral brow ptosis contributing to the appearance of upper lid heaviness, a lateral brow lift can be combined with upper blepharoplasty in the same session to avoid inadequate or asymmetric correction. Lower blepharoplasty is tailored to the anatomy of the individual lower lid. The transconjunctival approach — a concealed incision placed entirely on the inner surface of the lower lid — is used for patients whose primary concern is fat herniation without significant skin excess; it allows direct access to the three fat compartments of the lower orbit for conservative excision or repositioning without any external scar. Fat repositioning, where the prolapsed fat pad is draped over the orbital rim and secured to fill the adjacent tear trough, produces a smooth lid-cheek junction more naturally than excision alone. When lower lid skin redundancy or festooning (skin folds) is present, a subciliary external incision — placed just below the lash line — permits skin excision and orbicularis tightening in addition to fat management. Fat transfer from the abdomen or thigh can also be used to fill a deep tear trough or hollow lid-cheek junction in the same session. All blepharoplasty procedures at Rady Rahban MD are performed under anesthesia at the AAAASF-accredited Lasky Drive Surgery Center. Recovery involves mild swelling and bruising for two to three weeks, with most patients comfortable in social settings by the end of the second week.
