The transconjunctival approach — a concealed incision placed entirely on the inner surface of the lower lid — is used for patients whose primary concern is fat herniation without significant skin excess; it allows direct access to the three fat compartments of the lower orbit for conservative excision or repositioning without any external scar. Fat repositioning, where the prolapsed fat pad is draped over the orbital rim and secured to fill the adjacent tear trough, produces a smooth lid-cheek junction more naturally than excision alone. When lower lid skin redundancy or festooning (skin folds) is present, a subciliary external incision — placed just below the lash line — permits skin excision and orbicularis tightening in addition to fat management. Fat transfer from the abdomen or thigh can also be used to fill a deep tear trough or hollow lid-cheek junction in the same session. Recovery involves mild swelling and bruising for two to three weeks, with most patients comfortable in social settings by the end of the second week.
