Each pass creates thousands of microscopic perforations across the epidermis and into the upper dermis, initiating a localised wound-healing cascade: platelets aggregating at each micro-injury site release growth factors including platelet-derived growth factor (PDGF), transforming growth factor beta (TGF-β), and vascular endothelial growth factor (VEGF); fibroblasts are recruited from the surrounding stroma and activated to synthesise new extracellular matrix components; and the remodelling phase over the subsequent four to eight weeks produces new collagen and elastin deposition in the papillary and upper reticular dermis, measurably improving skin structure and resilience. This controlled fibrogenic response underlies microneedling's efficacy across a broad range of skin structure concerns — most notably atrophic acne scars of the rolling and boxcar subtypes, enlarged and visible follicular ostia, fine lines and early rhytids, rough or irregular skin texture, and post-inflammatory hyperpigmentation in skin types that respond poorly to ablative laser options. The micro-channels also create a transient increase in transcutaneous permeability, making simultaneous application of active serums far more effective than topical application alone. At SoBe Med Spa, a hyaluronic acid serum is infused through the open channels during each session to deliver deep dermal hydration and support the skin barrier during the recovery phase. Depth settings are adapted zone by zone: shallower settings of 0.5 mm for the periorbital region and superficial texture improvement, and deeper settings of 1.0–2.0 mm for the cheek and jaw regions where acne scar remodelling requires reaching the deeper papillary dermis. Topical numbing cream is applied 30 minutes prior to needling and is included in the 75-minute appointment window.
